According to local tradition, the church in Polana was built in the 16th or 17th century as a Roman Catholic court chapel. Then it was handed over to the Ruthenians by the village owner from the Urbańscy family in exchange for help in building the new church. This is how the temple was made of log, two-part structure, with an extended nave and a slightly narrower presbytery. The gable roof was topped with three turrets. In 1790 the temple was renovated and consecrated again. Another renovation, this time a thorough one, took place in 1922. The nave was extended and in 1937 a new polychrome was made. After 1951, the temple was abandoned by Greek Catholics. There was a grain warehouse in it for some time. From 1969, it was put into use of a Roman Catholic church. A two-story bell tower from 1922 with a pole structure stands next to the church. Nearby are also the remains of the former cemetery. The precious icon of Saint Nicholas from the church is today in the Sanok open-air museum. The entire historic complex is located on the Wooden Architecture Trail.
